Returning home after a stay in hospital can leave you and family members feeling worried.
We often support clients to recover and regain their confidence after a stay in hospital. Our team can provide as much or as little support as you need, perhaps round the clock care for a few days reducing to shorter regular visits over the following weeks.
Before you leave hospital, we can discuss with you and your family what help you may need and arrange for care to start when you are discharged. We will visit you at home to make sure that we are able to provide care for you in the safest way.

Personal Care - Our dignified and discrete approach
At some point in our lives, most of us will find it difficult to get dressed, undressed, or washed. Often family members want to be the primary carer for a loved one. However, assisting with dressing or washing can be dangerous if poor technique is used when lifting or moving.
Our experienced and trained care team will provide support safely and in a dignified way. Wherever possible our nurses and carers will encourage independence but will be there to assist if needed.
Our care team can take on as much or as little care as needed and for as long or as short a time as needed. They can also assist with medication, support to get in or out of bed, assistance with errands or support to go out during the day. Our team can also stay the night and assist if needed.

Taking medication in line with the prescribed instructions can be very important. Our approach is to support clients maintain as much independence as possible with respect to their medication.
Sometimes people require physical assistance only with medication for example: popping tablets from a blister pack, opening bottle tops or applying creams or eye drops. In these circumstances, our care team will follow our clients instructions allowing the client to retain responsibility. If a high level of assistance is required, we are happy to take that responsibility on.
We encourage all clients to have pharmacist filled packs rather than medication in individual bottles, this often allows clients to retain responsibility for their own medication for longer. We are only allowed to assist with medication which comes from a container filled directly by a pharmacist, we cannot assist with medication which is placed by a family member in a bowl or dosette box.
Our care team are more than happy to check that clients have taken their medication and prompt or remind if needed. Where we have concern about a client for example: not taking medication this will be reported to the clients care contact (family or doctor) and dealt with appropriately.

Home help - do you need help with tasks around the home?
For many, some help around the home is all that is needed to stay completely independent at home.

Your carer will be a friendly face that is glad to share a chat with you over a cup of tea or whilst they are helping you with household tasks. Companionship is an important part or our service and something that is really valued by our clients.
The next step is simply to decide what you want support with and how often you would like help. It is very easy to increase or change your support later. All our team are experienced and trained carers, if later you need support with personal care or medication, your existing team will be able help.
It can be difficult at first to accept support from other, often having some extra help at home is a great way to start.
Providing a friendly face and companionship is so important to our team and our clients – Jeevasare Health Care has always avoided and taken a stand against the dreadfully short visits that have plagued social care in recent years.
Our care managers take time to match nurses or carers with clients that will work well together, perhaps with shared interest or backgrounds. We always seek to ensure that our team has sufficient time to both support our clients with the help that they need and still enjoy a relaxed conversation. We also constantly work to maintain a small, regular team of nurses and carers for each client. By doing this, we facilitate the building of trust between care staff and clients something which often leads to genuine friendships.
Companionship can be key to maintaining quality of life, it is certainly important to providing care in a dignified way and support clients to lead lives that are as independent as possible.
